Care Coordination for Families in the Taos Region
Navigating healthcare, social services, and community resources can feel overwhelming, especially during pregnancy, after the birth of a child, or when a family is facing new medical, developmental, behavioral health, or social needs. Holy Cross Health offers free Care Coordination through the ROAMS program to help families in the Taos Region connect with the support, services, and resources they need.
The Holy Cross Health Care Coordinator, Lauren Bailey, supports perinatal individuals and families with children from birth through age 14. Lauren works with families receiving care through the Women’s Health Institute and the Taos Clinic for Children and Youth, helping ensure that families receive comprehensive, coordinated support during important stages of life.

What Does a Care Coordinator Do?
A Care Coordinator helps individuals and families identify their needs, understand available options, and connect with appropriate services. This support is provided free of charge and is designed to make local systems easier to access and less overwhelming.
Care coordinators assist families by:
- Helping navigate healthcare, social services, and community resources
- Connecting families with local providers, classes, programs, and support services
- Coordinating referrals and follow-up care
- Supporting families after major life events such as childbirth, a new diagnosis, or complex medical referral
- Helping families access services related to early childhood development, parenting, behavioral health, mental health, housing, and healthcare access
- Strengthening each family’s support system beyond family and friends
Care Coordination is especially helpful for families who may be unsure where to start, who are managing multiple needs, or who need help understanding what resources are available in the Taos area.
Coordinated Support Across the Community
Lauren works closely with a strong network of local partners to help families receive well-rounded support. These partners include First Steps Home Visiting, Taos Loves Kids, Lactation Specialist Jana Bailey, the Holy Cross Labor and Delivery team, La Luz, Los Angelitos, UNM-Taos, and Youth Development, Inc.
Through this collaborative approach, families can receive support from pregnancy through adolescence, with continuity of care at each stage. Lauren has helped families connect with resources such as Social Security benefits, private therapy services, parent education programs, home visiting services, lactation support, and other community-based programs.

Care Coordination can help families feel more stable, informed, and confident as they access care and community support. When services are coordinated early and consistently, families are better able to build a sustainable support system and receive the help they need when they need it.

Whether a family needs help finding services, understanding referrals, accessing support after childbirth, or navigating challenges related to parenting, development, mental health, housing, or healthcare, Care Coordination can provide a helpful starting point.
