Community Health Needs Assessment Survey

Holy Cross Medical Center is conducting a Community Health Needs Assessment (CHNA) to evaluate the overall health needs of our region. We are inviting you to participate in a brief survey to help guide this work.

Your anonymous response will help us understand diverse community perspectives, prioritize the health needs in our area, and develop an action plan to address those needs. This survey should take less than 15 minutes, and results will be available in several months at the completion of our assessment. Please complete the survey by January 31st.
